The Collaborative
Improving Healthcare Access Through Regional Collaboration
The Yuba-Sutter Regional Healthcare Collaborative is a regional partnership led by the Yuba-Sutter Healthcare Council & Foundation (YSHCF) that brings together healthcare providers, clinics, community organizations, public health agencies, hospitals, funders, and residents to improve health outcomes and expand access to care across Yuba and Sutter counties.

Guided by the YSHCF’s mission
“To bring together healthcare and key stakeholders to leverage community resources to encourage and promote a healthy region"
The Collaborative focuses on practical, data-informed solutions that address real barriers facing our communities
Why This Work Matters
Residents in Yuba and Sutter counties face shared healthcare challenges, including:
-
Limited access to specialty care
-
Transportation and geographic barriers
-
Digital literacy and internet access gaps
-
Long wait times and complex referral pathways
By working regionally, partners are able to coordinate efforts, share data, and develop solutions that improve access while reducing duplication.
What We've Done
-
Hosted the first Conversations in Healthcare Town Hall in 2024, with over 75 residents sharing their healthcare experiences and priorities
-
Conducted a regional specialty telehealth study in 2025, gathering insights from:
-
300+ patients served by local FQHCs
-
Primary care providers and referral staff
-
-
Identified key barriers and opportunities related to telehealth access, patient
-
readiness, and provider workflows
