Your Health Matters to Us

The Yuba-Sutter Healthcare Council & Foundation (YSHCF) is a nonprofit organization dedicated to improving healthcare access and outcomes across the Yuba-Sutter region. YSHCF collaborates with local healthcare providers, community organizations, public agencies, funders, and residents to facilitate easier access to and navigation of healthcare services. Through its leadership of the Yuba-Sutter Regional Healthcare Collaborative, YSHCF brings people together to listen to community experiences, share information and data, and work toward practical solutions that address regional healthcare challenges and reflect the needs of residents across Yuba and Sutter counties.

Implementation Timeline

Initiatives

Goal 1

Conduct discussions and begin development for the expansion of telehealth services.
Collaborate with the Regional Healthcare Partnership to host a Regional Healthcare Summit.
Secure commitments from both Yuba County and Sutter County Health and Human Services.

Goal 2

Obtain MOU and program approval for the Community Opioid Prevention Network (COPN).
Continue implementation of the COPE (Community Overdose Prevention Efforts) program, including monthly progress reports and financial invoicing.
Hold bi-monthly COPE team meetings and participate in the Yuba County Opioids Task Force every month.
Submit COPN Annual performance reporting every year in Q4.

Develop and implement a new Government Relations Committee.
Develop a multi-sector alliance structure.
